EMERGENCY HVAC SERVICES REQUEST
Thank you for allowing Garden State Air, Inc. to provide you with Emergency H.V.A.C Services during your unfortunate predicament. We here at GSA do service this type of emergency situation often and things do not always go as planned.
Please understand that if you choose to cancel your order after our truck has been loaded and left this facility, you will be responsible for 100% of the Emergency Service FEE.
The emergency service rate is: $____________95.00_________________________ (FREE Service for Platinum Contract Holders)
This rate does not include parts or labor, this is a after hours service diagnostic fee. (6 p.m. - 8 a.m. Monday - Saturday, All Day Sunday)
(155.00 emergency service fee applies on holidays)
FREE Service Call with Repair (Monday - Saturday 9 a.m. - 5 p.m.)
If you agree to these terms, please submit this form as soon as possible. We are unable to send our truck out until we have received this agreement.
